MY DETAILS?
Please inform the receptionist
if you change your name, address or telephone number. We must have your telephone
number even if it is ex-directory so we can contact you in an emergency. It will
be kept strictly confidential. If you move out of the practice area, you will
be required to find a new doctor.
HOW
DO I... MAKE AN APPOINTMENT?
You may make an appointment by calling in at the reception desk or telephoning
the receptionists on (0115) 950 1854.
We will do our best to keep to the appointment time, but emergencies do occur.
Children should be accompanied by an adult.
If you need to see the doctor urgently, please explain this when you telephone
or call at the surgery.
If you have two problems or a complicated problem please book a double appointment.
Each appointment is for one person only. Please book separate appointments for
each person.
Most of the GPs are happy to carry out telephone consultations. Please ask at
reception for details.
Please note: Dr Smith and Dr Taplin will not deal with the termination of pregnancies.
Please make an appointment with one of the other GPs.
If you cannot keep your appointment, please let us know in advance so that it can be offered to someone else.
ACCIDENT
OR EMERGENCIES
During Surgery Hours - (0115) 950 1854
If you have an emergency, telephone the usual surgery number. Genuine emergencies
will always be given an urgent appointment, although if no doctors appointment
is available you will be given an appointment with one of the practice nurses
who will see your problem is dealt with appropriately.
Outside Surgery Hours - (0115) 950 1854
If you or your family need URGENT medical care when the surgery is closed, please
telephone the usual daytime number. Your call will be automatically diverted
to the out-of-hours emergency service.
You will be asked for details of the patient and their condition and will then
be:
• given advice, or
• arrangements will be made for you to be seen by a healthcare professional
or
• offered a home visit.
The out-of-hours service is for emergencies only.
Deaf patients requiring an emergency doctor can textphone NHS Direct for a doctor
on 0800 606 4647.
ALTERNATIVES
Why wait for a doctor’s appointment?
With the Pharmacy First Scheme you may be able to get appropriate
treatment from your pharmacy sooner.
If you are registered with a doctor in Nottingham City and are exempt from prescription
charges you can register at a participating pharmacy and still obtain free of
charge medication for certain ailments. (See separate poster in reception for
more details and participating pharmacies or ask at reception.)
For advice you could also contact NHS Direct on 0845 4647.
For minor illnesses and injuries visit the Walk-in Centre on London Road, Nottingham. This opens daily until 9.00pm, 365 days a year.
HOW
DO I... OBTAIN A HOME VISIT?
Home visits are made for those patients who, because of their condition, cannot
get to the surgery. If you would like to discuss if a visit is necessary please
telephone the reception staff on (0115) 950 1854 before 11.00am whenever possible.
Late requests will mean the visit may not be made until the next day.
Please remember that several patients can be seen at the surgery in the time
it takes to do a home visit.

DO I... OBTAIN A REPEAT
PRESCRIPTION?
If you are on regular long-term
medication, you will be issued with a computer-printed prescription. It is likely
you will be asked to see a doctor or practice nurse at regular intervals so your
condition can be reviewed. You will be given a computer list of your medication
to make reordering easier.
We require 24 hours’ notice for repeat prescriptions.
• Prescriptions can be ordered at reception; we also accept fax and postal
requests.
• Telephone requests may be accepted after 1.00pm. You may be asked to
call back if the reception staff are extremely busy.
• If you would like your prescription posted to you, please enclose a stamped
addressed envelope.
• Order early to allow for possible postal delays, especially at times
of bank and public holidays.
• Most pharmacies operate a prescription order and collection service; ask
for details at your local chemist.
Please let us know if you have had your treatment altered following a hospital
visit, as they do not always inform us.
On-Site Pharmacy
We have Collins Pharmacy (Nottingham) Ltd, T/A Turners Pharmacy on-site, occupying
purpose-built premises at the surgery and providing pharmaceutical services.
These include dispensing prescriptions, provision of advice on minor ailments
and supplying over-the-counter medicines under the Pharmacy First scheme. A medicines
use review is available to patients with chronic illnesses and on long-term medication.
They also offer a prescription collection and delivery service.
HOW
DO I... REGISTER WITH
THE PRACTICE?
You will need to present your signed medical card to a member of the reception
team. If this is not available there is a simple form that will need completing.
An appointment for a new patient check will then be arranged with one of the
practice nurses. Please bring along any medication that you take. If registering
a child you will also need to present a record of any immunisations given. This
is normally in the red book.

DO I... OBTAIN TEST RESULTS?
Please telephone the surgery after 1.00pm to obtain the result of any test you have had done.
